Enquiry privacy notice

This notice covers the initial enquiry forms on the Up Health Travel website. It explains the information requested at this stage and the purpose of collecting it.

What the forms collect

The trip planner collects your name, email, optional phone number, contact preferences, broad planning goal and travel preferences such as timing, budget and group size. The contact form also collects your question. Please do not include medical history, symptoms, diagnoses, results, medications, passport details or payment information.

Why we collect it

Your information is used to review and respond to your enquiry, discuss possible trip arrangements and manage the planning conversation. Submission does not subscribe you to marketing and does not authorise a booking.

How the forms are handled

Submissions are processed and stored using Netlify Forms, our website form service, and are available to authorised administrators of the Up Health Travel project. Service-provider infrastructure may process information outside Australia. See Netlify’s privacy policy for information about its handling of data. Avoid sending sensitive details through these forms.

Providers and travel suppliers

An initial enquiry does not authorise sending your medical information to a provider. Any later booking process should identify the suppliers involved, the information they need and the applicable privacy notices before you proceed. Share clinical information directly through your medical provider’s secure process.

Access, corrections and privacy questions

Use the contact form and select “Privacy or my information” to request access, correction or deletion, withdraw permission for further enquiry contact, or raise a privacy concern. Include only enough information to identify your enquiry. We may need to verify your identity before acting on a request.

Website services

This website uses Netlify hosting and Google Fonts. These providers may receive technical information such as an IP address when the website loads. We have not added advertising trackers or marketing analytics. Information typed into the trip planner is not saved in your browser between visits.

Before you become a client

Further information about business contact details, retention, supplier arrangements and any additional personal information required must be provided as part of onboarding. This website enquiry is not a medical intake form.
